Patient Rights and Responsibilities
Neurological Monitoring Associates, L.L.C. (NMA) is committed to providing each of our patients with equal rights and responsibilities to facilitate a collaboration towards the highest quality outcomes. Your active involvement in your health includes understanding these rights and responsibilities. We encourage you and your family members to be actively involved in your treatment options, and to help promote your safety by being informed and actively involved in all aspects of your care.
Your Rights:
- You have the right to seek consultation with the physician(s) of your choice.
- You have the right to receive care in a safe environment, which is free from all forms of abuse, neglect, or mistreatment.
- You have the right to be addressed by your preferred name and in a dignified manner at all times.
- You have the right to be communicated with in a language that you can understand. A foreign or sign language interpreter will be provided at no cost to you as needed. Information provided to you will be appropriate for your age, language, and understanding. Additional aids will be utilized if you have vision, hearing, or other impairments to ensure your needs are fully met.
- You have the right to know the names of all health care team members who are providing or directing your care.
- You have the right to be informed about the benefits and risks of monitoring, as well as any expected or unexpected possible outcomes.
- You have the right to be treated confidentially, with access to your records limited to those involved in your care or as designated by you.
- You have the right to agree, or refuse, to take part in medical research studies.
- You have the right to refuse visitors at any time.
- You have the right to refuse medical treatment or procedures.
Your Responsibilities:
- Provide complete and correct information, to the best of your ability, about your health
- Inform your health care team of any changes in your health which could impact treatment
- Be respectful to and support our healthcare professionals and staff
- Ask questions regarding areas of concern
- Voice complaints, concerns or suggestions to a member of the NMA team
